Tezuka was probably basing the idea on old stories about male eunuchs having a tendency to become feminized. And as we've seen in a few Black Jack stories, although he may have a medical license, he is also unafraid to do stories on medical conditions that fall outside his specialty and that he actually knows fairly little about.
I mean, he once wrote a story about a kid getting flashbacks from someone else's death because of cellular memory from a skin graft. Its awesome, but totally bullshit. :P
